Overview
All the forms controlling members may need are listed below with a short description. It is extremely important that all applicable fields in the forms be completed and that all required documentation is enclosed. If the business manager sends in an application for a security business licence and it is incomplete, including incomplete information from any controlling member, the process will be delayed or possibly returned unprocessed.
Controlling members who DO HOLD a current B.C. security worker licence –
just give the business manager the security worker license number.

Authorization and Acknowledgement of Information and Use AND Consent to a Criminal Record Check
Use this form if: you are a controlling member of a business applying for a security business licence and you DO NOT HOLD a current Security Worker Licence. This form, when completed and signed, will provide the Registrar with your personal information, your acknowledgment that you will comply with conditions set out in the Security Services Act and your consent to a criminal, police and correctional record search. It is mandatory that all controlling members complete this process. |

Controlling Member of a Security Business Reporting an UPDATE
Use this form if: in compliance with the Security Service Act, you are reporting updates to your records held by the Registrar: change of residential address, new criminal charges or convictions, change in peace officer status, a treatment of a mental condition, or a change in personal legal name. |

Mental Condition Form
Use this form if: you have a mental condition that must be reported to the Registrar. It will be completed by your doctor and sent to the Registrar for review. |

Request for Fingerprinting
Controlling members who do not hold a security worker licence must have their fingerprints taken as part of the process for the business to obtain a security business licence. Instructions are in the ‘Guide’ and on the form itself |
